We cannot replace your lost or misplaced COVID-19 Vaccine Card but we can provide you with an official immunization record that documents that you received the COVID-19 vaccine free of charge! To access the Michigan Immunization Portal, you must be a Michigan resident age 18 and older.
Individuals will be asked to create a MILogin account and upload a valid government-issued photo ID. Our call center is also available to assist in helping residents get a record of their immunizations.
